COVID-19 Parent Acknowledgement Form

By signing below, I acknowledge that I have reviewed the School’s Covid-19 Safety Plan, and agree that my child(ren) and I will adhere to the guidelines and protocols set forth in that document, as well as any other expectations established by the School, and that failure to do so may result in appropriate consequences, as determined by the School. I agree that I will keep my child(ren) home if they are sick and I will always provide candid and truthful information. I acknowledge that a failure to provide candid and truthful information, or any other action that is contrary to the guidelines and protocols in the Covid-19 Safety Plan and/or the overall health and safety of the Diener community, may result in appropriate consequences, as determined by the School. I further acknowledge that I understand the risks inherent in my child(ren) attending school during the 2022-2023 school year, and that by sending my child(ren) to the School I accept those risks on behalf of myself, my child(ren), and all household members. By signing below, I acknowledge and agree that my electronic signature below has the same legal effect and validity as a written signature and that this Acknowledgement is valid and will be given the same legal effect as a written and signed Acknowledgement.
